“Tarefeiro” alarm before winter in SNS emergencies: Why are hospitals dependent on these doctors?

The attempt to save the emergency departments of public hospitals in Portugal from dependence on external doctors working on hourly contracts has turned into a new debate as the winter period approaches.

Hospital administrators warn that filling some emergency room shifts could be very difficult during the winter months without provider doctors, often referred to as médicos tarefeiros in Portugal.

On the other hand, the government is trying to reduce the cost of this system, which has been discussed for years, and its weight on SNS.

What is a “Tarefeiro” doctor?

These are generally not staff doctors of SNS hospitals.

Hospitals can purchase external doctor services, especially in emergency departments, for shifts that they cannot fill with their own staff.

Physicians work on an hourly rate and, in some cases, can earn significantly more per hour than staff SNS physicians.

The system allows hospitals to fill open shifts in the short term.

However, in the long run, SNS has been criticized for years on the grounds that it makes it difficult for it to create its own physician staff and creates high costs for the public.

Why is winter critical?

The increase in flu, Covid-19 and respiratory infections during the winter months in Portugal increases the pressure on emergency services.

Therefore, it is important for hospitals to cover gaps in doctor shifts, especially during this period.

If a significant portion of Tarefeiro doctors withdraw from the system, it may become difficult to fill shifts in some emergency departments with existing staff.

Why do doctors react?

The body representing Tarefeiro doctors opposes serious reductions in the new hourly wage system prepared by the government.

In previous statements, the possibility of large-scale work stoppages or denial of service was raised if wages were significantly reduced.

But there is an important distinction here:

As of today, there is no national tarefeiro strike declared across Portugal.

The discussion is on how the new fee schedule will be shaped.

Government's dilemma

There are two sides to the problem.

Overuse of the Tarefeiro system increases the cost of SNS and can undermine staff physician recruitment.

However, shrinking the system too quickly may cause shift problems, especially in emergency departments where there is a shortage of doctors.

Therefore, for those living in Portugal, the main issue to be monitored is not how much doctors will charge, but whether the new model will affect emergency rooms remaining open during the winter period.
